Goal

Make the builder available where users write emails.

Requirements

Templates hold the theme plus a header and footer built with the visual builder.
Campaigns get a 'Visual' editor type next to Markdown, HTML and plain text.
A Visual campaign that uses a template shows the template header and footer around the campaign body and uses the template theme.
Existing Markdown, HTML and text campaigns and templates keep working unchanged.
Switching a campaign away from Visual warns that the visual layout will be lost.

Acceptance

A user can create a template with a theme, header and footer, then create a Visual campaign from it, preview it, send a test and send it.
Existing campaigns and templates open and send exactly as before.
