Part of #19.

Goal

Turn the block document into HTML that renders reliably in common email clients, and preview it while editing.

Requirements

Table-based layout with inline CSS that works in Gmail and Outlook (including Outlook desktop on Windows).
Columns stack on narrow screens where the client supports it, and still read well where it doesn't.
Buttons render as clickable buttons in Outlook.
Live preview with desktop and mobile width toggles.
Existing placeholders keep working in the generated HTML.
A plain-text part is still generated for every send.
The JSON document is the source of truth; HTML is generated from it on save and send.

Acceptance

A sample email using every block type renders correctly in Gmail (web and mobile) and Outlook.
Placeholders are replaced in test and real sends.
Sent messages contain both HTML and plain-text parts.
