Add named, manually-managed Contact Groups alongside rule-based Segments.

Prisma Group model (project-scoped) and ContactGroup join table (many-to-many) with addedAt.
Cascade delete of memberships when a contact or group is deleted.
Group CRUD API; name unique per project; optional description.
List/detail responses include member counts.
Tests for CRUD, uniqueness, cascade and project isolation.
